- A signed Assumption Of Risk Waiver Form is required for all members and guests before using the Sports Center's indoor and outdoor facilities.
- A parent or legal guardian must sign the form for youths under the age of 18.
- Forms can be found at the Main Desk. If needed, completed and signed forms may be faxed to us at (607) 547-4100.
